Dr. Truong and his team were really great! I went in there knowing I wanted to do something on my stomach, which is hard to decide with so many procedures out there, and they did a great job explaining to me what procedures were available and what would be the best for me, and most importantly, making me feel comfortable. Also, Monika, the patient coordinator, was beyond amazing. She really helped me understand the pros and cons of the procedure and was available to answer all of my questions at different hours. She really earned my trust, which is important when one really wants to hear the truth about the procedure, recovery, and deciding if they should do it or not. She was so attentive that she even texted me after to see how I was doing. When one decides to have such a big procedure done, its important to feel comfortable and that you are somewhere where you trust the Dr. and staff. From my first consultation, I knew Dr. Truong would be an outstanding doctor. What is Lipo 360?

Liposuction of the entire mid-section

Lipo360 is a relatively new term in liposuction. It refers to fat removal of the entire trunk or mid-section of the body which includes the upper and lower abdomen, the flanks or sides, and the back area.

In other words, fat is being removed in those areas circumferentially in a 360 degree fashion – hence the term. The concept of performing liposuction in these areas is not new, however the artistic philosophy and techniques in 360-degree liposuction have been refined by surgeons who perform liposculpting.

The term Lipo 360 was first popularized by surgeons performing the procedure to harvest fat for autologous fat transfer procedures like the Brazilian Butt Lift. At Chicago Breast & Body Aesthetics, our surgeons perform Lipo 360 using minimally-invasive Tumescent Liposuction combined with Power-Assisted Liposuction. Lipo 360 can be performed as a stand-alone procedure, or in conjunction with Brazilian Butt Lift (BBL). Often other areas are treated at the same time including thighs, hips, and arms.

What is “tumescent liposuction”?

Tumescent liposuction refers to liposuction where tumescent solution is used, which is how most liposuction procedures are performed. Tumescent solution is sterile saline solution which contains medications to help make liposuction safer and more tolerable.

These medications often include lidocaine (a numbing agent) and epinephrine (a blood vessel constricting agent), and bicarbonate (an acid neutralizer). Bathing the fatty tissues with saline solution helps to wet and disperse the fat allowing for easier removal with small cannulas. The lidocaine helps to make the procedure less painful, while the epinephrine significantly reduces the amount of bleeding that can occur, and the bicarbonate makes the solution less acidic.

The most commonly used tumescent solution is Klein’s solution which was developed by dermatologist Jeffrey Klein in 1987. Each one liter of Klein’s solution consists of 0.9% Normal Saline, 100ml of 1% lidocaine, 1ml ampule of 1:1,000 epinephrine, and 10ml of 8.4% bicarbonate. Today there are many variations on Klein’s solution but the basic principle is the same.

How is Lipo360 performed?

Minimally-invasive & surgical

Lipo360 is a minimally-invasive surgical procedure which means the procedure is performed using small instruments and very tiny incisions. This procedure can be performed using local anesthetic, IV sedation, or general anesthesia.

Once you are anesthetized, your surgeon will make a very small (typically 2mm) incision in well-hidden areas. Through that incision, the tumescent solution is injected to wet the fat and anesthetize the area. Once the area is adequately medicated the treatment begins.

If a laser-assisted device like SmartLipo® or ultrasonic Vaser® is used, it is done at this time. Next, a long thin tube called a liposuction cannula is carefully inserted to safely remove the fat using a vacuum device called an aspirator.

The amount of fat and the areas treated are determined by the surgeon using artistic and medical judgement. Once the fat is removed evenly, the incisions are usually closed with a single stitch. You will then be placed into a compression garment which will be worn during the recovery period.

What is power-assisted liposuction?

Power assistance

Power-assisted liposuction or “PAL” refers to liposuction in which a special device is used to move the liposuction cannula at a high rate to aide the surgeon in removing fat more effectively. Think of it as a power tool for liposuction! At Chicago Breast & Body Aesthetics, our surgeons use the Microaire ® PAL Lipoculptor®, with is the only U.S.-patented liposuction instrument using a reciprocating motion (rather than rotating) to facilitate the movement of the cannula within the tissue.

Am I a candidate for Lipo 360?

It depends on these factors:

If you have excess fat around the abdomen, waist, and back areas then you are potentially a candidate for Lipo 360. Ideal candidates for this procedure are patients in good general health, and are at or near their ideal body weight. Liposuction is designed to treat and remove subcutaneous fat only. Patients who have loose skin, loose muscles, or visceral (intra-abdominal) fat may need other procedures like a Tummy Tuck or diastasis repair instead of liposuction alone. Also patients who have abdominal hernias should have their hernias fixed prior to having liposuction.

How much weight will I lose with 360 liposuction?

Liposuction is not a weight-loss procedure

Liposuction is not a weight-loss procedure, it’s a sculpting procedure. Patients who are obese or who have a BMI over 30 may need lose weight first in order to be considered for Lipo 360. The maximum amount of fat that can be removed safely during liposuction is 5 liters which only amounts to around 10lbs.

However, the scale doesn’t tell the whole story. Because the fat is selectively removed in the problem areas in the midsection, patients will have Lipo360 will have the appearance and benefit of a much more significant weight loss. Also most patients who maintain a healthy diet and exercise regime postoperatively will continue to lose weight after liposuction due to heightened metabolism and decreased ability to store fat in the treated areas!

Is liposuction permanent?

Yes!!

Yes, liposuction is a permanent procedure. You only have a set number of fat cells in your body by age 13 or and fat cells don’t usually re-generate. This means that once they are removed they are usually gone for good! However, since fat cells store fat, the ones that remain can still enlarge if you don’t maintain a healthy diet and exercise routine.

If you gain a significant amount of weight, then you will make those remaining fat cells larger and you may reverse the benefits of your liposuction procedure. Patients who have the best long-term results from liposuction are the ones who maintain a stable healthy weight or lose weight following the procedure.

Is liposuction safe?

Risks of serious complications are very low

Yes, when performed correctly liposuction is a very safe procedure. This means that liposuction should be performed by a properly trained and experienced, board-certified plastic or cosmetic surgeon who has done many of these procedures. It should also be done a licensed and accredited surgical facility with proper staff and sterile instruments. When done in this manner, the risks of serious complications are very low.

When performed appropriately and in the appropriate individual, there should be no real long-term side-effects from liposuction. In the immediate post-operative period, you may experience some discomfort or pain, swelling, bruising, and numbness or tingling in the treated areas. Most of these symptoms will resolve within a few weeks. Numbness and tingling can sometimes last for many months, but are almost always temporary.

If a significant of fat is removed from the abdominal area where hormones are stored, patients may experience some temporary fluctuations in hormones but this will typically normalize within a few weeks. Other than these temporary effects, there should be no long-term negative effects from having liposuction.

Will the fat move to other areas after liposuction?

This is a common misconception

This is a common mis-conception. The body only has a finite number of fat cells and once they are removed with liposuction, they don’t usually come back. Also fat cells don’t move or migrate so it is impossible for the fat cells to move to other areas of the body.

More commonly, patients who gain weight may notice untreated areas getting disproportionately larger than areas where liposuction was performed because now there are fewer fat cells in the treated areas. This is the reason we recommend our patents to maintain a healthy diet and exercise routine to prevent weight gain following liposuction.

Is liposuction painful?

It is surgery, so some pain should be expected

Liposuction is a surgical procedure, so you have to expect some discomfort during the immediate recovery period. Most patients describe the feeling as soreness and stiffness similar to a very intense work-out. For more severe discomfort, pain medication will be prescribed. Most patients don’t require narcotic pain medication for more than a few days, and many patients only need Tylenol.

What is recovery like after liposuction?

The recovery after liposuction is generally well-tolerated. Immediately after your procedure you may feel some discomfort or pain similar to a very intense work-out. There is usually a significant amount of draining and leaking from the incision sites which usually stops after a few hours. You should also expect to have some bruising which should resolve after a week or two. Most of the swelling will resolve in a few weeks, but some swelling will last for many months.

As the tissues heal, scar tissue will form which can cause a feeling of hardness or tightness which can last for many months but should return to normal by 6 months. Numbness can last for several months as well but typically will return to normal given enough time. Other than the first day, you should not be be-ridden following liposuction and most patients are up and moving about the same day or next day after their procedure.

There are no diet restrictions and you are encouraged to drink plenty of fluids. Most patients are back to work in less than a week depending on their job. You should be able to resume exercising in 2-4 weeks. You will be asked to wear a compression garment for at least 1 month after the procedure to help with swelling and to maximize your results.

Each patient will receive a quote at their consultation which will delineate all of the costs of the procedure including the surgeon’s fee, anesthesia, facility fee, medications, garments, and lab tests if needed.
